The Vermont Department of Disabilities, Aging and Independent Living (DAIL), Developmental Disabilities Services Division (DDSD) is partnering with service providers to explore new payment models for developmental disabilities services.

The Burns & Associates division of Health Management Associates (HMA-Burns) is assisting DDSD with this initiative and has established this website as a repository for study-related materials.

 PROVIDER SURVEY

A key factor in this effort is a provider survey to collect data regarding providers’ service delivery designs and costs, which is available for download:

• A webinar that walks through the survey can be accessed here.

Data collected through this survey will be used solely for the purpose of evaluating payment reform options. Only aggregated data will be reported; no provider-specific information will be published.
